and Withdrawal & Refund Policy


TERMS OF ENROLLMENT

1. I understand that the Registration Fee is Non-Refundable

2. I understand that there are no refunds on tuition unless student has been hospitalized for a serious illness of injury. Further, if student wishes to discontinue, he or she must submit a Withdrawal Form (available in the office).

3. I understand that, in participating in any physical activity, there is a risk of injury. I understand that The Studio of Dance & Music assumes no risk whatsoever, including but not limited to, the proprietor’s own negligence. I further state that the enrolled student is fit and has a physician’s approval to participate in this type of activity. This statement constitutes an express assumption of risk on behalf of my/my child.

4. I understand that there are no refunds on missed dance classes. Further, I understand that missed dance classes can be made up by attending another class recommended by the teacher.

5. I understand that no make up music lessons will be given unless the student’s absence is due to a serious illness AND a six (6) hour notice has been given to the school office. Further, I understand that make up lessons will be LIMITED TO 2 PER SCHOOL YEAR and that there are NO REFUNDS. (School activities or trips, sports games, vacations that differ from the Studio’s school year calendar, etc. do not qualify for make up lessons.) I further understand
that if the teacher cancels a lesson, I am entitled to a make up lesson scheduled at the student and teacher’s mutual Agreement, and arranged by the studio office.

6. I understand that my payment is due in a timely manner in accordance with the Studio’s payment schedule. I understand that after a 7 day grace period a $10.00 Late Fee will be posted to my account. I further understand that, if at anytime during the enrollment period, my account is 30 days or more past due, the enrolled student will not be eligible to attend class.

7. I understand that I will receive my dance recital costume (s), the FREE Recital Ticket for registration fees paid, and be allowed to participate in the end of year performance only when all fees due to the Studio (Registration Fee, Tuition, Late Fees, Costume Payments, Bounced Check Fees, etc.) are PAID IN FULL. I understand that, once costumes have been ordered, I agree to pay the entire costume fee even if I decide not to participate in the recital.

8. I understand the Studio rules and dress code and promise that I/my child will adhere to them. I understand that any violation of the rules and/or behavior that is destructive or detrimental to studio property or to other students may result in my/my child’s dismissal.

9. I understand that recital photos and other in studio photographs may be placed in ads
and other promotional materials, or on line at the studio web site, www.thestudioofdanceandmusic.com.

WITHDRAWAL & REFUND POLICY

1. If paying monthly for music lesson, on the 8 payment or 4 payment plan for dance classes, a student may withdraw from a class by completing a Withdrawal Form (available in the office). All students must submit the Withdrawal Form two weeks prior to the next tuition due date. Upon the timely receipt of the Withdrawal Form, the Studio will discontinue charging your credit card or return your unused post-dated checks and will discontinue tuition charges.

2. If paid for the full school year, a student may withdraw from a class by completing a Withdrawal Form and submitting it to the office. The Studio will retain the monthly tuition fee for all full and partial months of enrollment prior to the receipt of the Withdrawal Form. A pro rata refund for the balance of the school year will be issued within 30 days of receipt of the Withdrawal Form.
